1. From your Jolt Web Account, access Content > Lists and hit +New List. The List Name can be Employee Write-Up, Disciplinary Notice, or Corrective Action.
2. Display as a Form
3. For Role-based Access check the Assigned and Manage columns for roles starting at the Manager level and higher.

4. Click +New Notification. Select Manager and any other Role that you would like. Send an email when the List is completed.
5. Hit +New List Item and repeat this for the following items.
a. Employee Drop Down > Prompt Text = Employee who is receiving corrective action.
b. Multiple Choice > Prompt Text = What Type of Warning is this? -> Add Choices = Verbal Warning, Written Warning, and Suspension
c. Date/Time > Prompt Text = When did the incident occur?
d. Free Response > Prompt Text = Description of the issue.
e. Free Response > Prompt Text = Describe corrective action to be taken by the employee being written up.
f. Free Response > Prompt Text = Consequences for employees failure to improve performance.
g. Free Response > Prompt Text = Employee Comments.
h. Signature > Prompt Text = Employee Signature.
i. Signature > Prompt Text = Manager Signature
