1. From your Jolt Web Account, access Content > Lists and hit +New List. Name the list Maintenance Request.
2. Display as a Form.
3. Click +New Notification. Select Maintenance and any other Role that you would like. Send a text and email when the List is completed.

4. Click +New List Item and repeat this for the following items.
a. Multiple Choice:
i. Prompt Text = What Type of Maintenance is needed?
ii. Add Choices

b. Free Response:
i. Prompt Text = Describe what the problem is.
i. Prompt Text = Take a photo of what needs to be fixed.
